    ASSOCIATE VERIFICATION

    On March 13, 2020, the federal government designated COVID-19 as an emergency order under the Robert T. Stafford Disaster Relief and Emergency Assistance Act (the “COVID-19 Disaster”).  Pursuant to the Dean and Barbara White Family Foundation Disaster Relief Program, I certify that the Disaster Relief payment that I am requesting does not exceed, after taking into account other money available to me from the CARES Act, insurance, unemployment benefits, other federal, state or local aid, charitable aid, or otherwise, the amount I need to pay:

    1. reasonable and necessary personal, family, living, or funeral expenses incurred following the COVID-19 Disaster; plus

    2.  reasonable and necessary expenses incurred for the repair or rehabilitation of my personal residence, or repair or replacement of its contents, or other unforeseen additional costs incurred as a result of the COVID-19 Disaster.

    Based upon the foregoing, I understand that this payment will not be included in my gross income for federal income or employment tax purposes.
